Gastroesophageal Reflux disease (GERD) has recently been added to the list of suspected autoimmune diseases. See: “Gastroesophageal Reflux Might Cause Esophagitis Through a Cytokine-Mediated Mechanism Rather Than Caustic Acid Injury”, Rhonda F. Souza et al. Gastroenterology 2009, 137, 1776–1784.
Might be worthwhile to do a strict paleo diet with no nightshades, pseudocereals (amaranth, buckwheat, quinoa, etc…), eggs, quillaja, and alphalpha sprouts.

I have had GI issues pretty consistenly over the last 5 years, most recently diagnosed with GERD. I was on Acifex and a steady diet of antacids. I started the Paleo diet for weight and fitness reasons 3 months ago and my GERD is completely gone. I stopped taking the meds and havent popped an antacid in a long time. The only time it flares up is when I cheat and eat some wheat products. My mother and father in law have their own GI issues (gastritis and GERD) and have tried some wierd chinese holistic diet but refuse to give up the grains. Some people just refuse to believe, regardless of the science or the results.

I had a similar experience. I do not notice gluten type problems with beans, rice and corn but if I eat them consistently they DO cause GERD.

1) Have you already answered Kyle’s question re: phytic acid in nuts (in your comments http://robbwolf.com/2010/02/08/nutrition-epigenetics-pregnancy-and-postpartum/), also interested to hear about it but maybe it’s just my inability to find the solution in your blog/show?! Used the search but did not gather.
2) Do you have anything to say, one way or another, about using the antihistamins for allergies (such as pollen)? It’s that time a year in Finland and tho I’m not in need of those, a lot of people sneeze and have itchy eyes and are forced to ear Histex/Zyrtec (in Finnish: setiritsiinidihydrokloridi) quite a long time… Don’t know whether this is your expertise but am gonna ask, still.

I think it’s GERD that I am suffering with. Basically, feel like really bad heartburn. It causes me to vomit especially when I excercise and has sort of a gag effect when I brush my teeth even. I am not 100% paleo but pretty close and still suffering. Doctor just says prilosec every morning and he’s right it takes it away but do I understand that the Prilosec is bad for me? If so, what is the negative with it? I have an annual check up next month and want to be able to discuss with doctor where to go with this? In the meantime, I will do a strict 30 day paaleo and see what happens. Thanks for everything you both do!

I’ve heard that L-carnitine is not required to shuttle medium-chain triglycerides into mitochondria before. However, I have not been able to find any references. If anyone knows where to look, please point me in the right direction.
Alternatively, I have found evidence that L-carnitine serves a role in the metabolism of medium-chain triglycerides, see: “Role of Carnitine in Utilization of Dietary Medium-Chain Triglycerides by Term Infants”, Rebouche, Charles, J; Panagides, Dora, D.; Nelson, Steven, E.; American Journal of Clinical Nutrition 1990, 52, 820–824.

“Spinning out” as you so eloquently put it, seems to buggar several people I know, including myself. Next thing you know that ounce of dark chocolate you promised you’d limit yourself to has turned into the raiding your neighbor’s pantry. Is there anything we can consume, stay away from or do to mitigate or minimize this? I’ve heard magnesium might help. I know sleep is huge, along with monthly cycles and of course, stress is a given. I’m in Iowa and we’ve had the harshest winter documented since I’ve been alive. I feel your pain regarding the lack of sunshine. We were all about to go postal here but we’re feeling much better now. Robb – your comments on hypoglycemia were interesting to me. I suffered for years from hypoglycemia and found that going Paleo obviously solved the issues, the expermentation for me for when i tried to train with professional bodybuilder whose answer was numerous amounts of protein and carbs and shakes and tons of food all the time. which limited the episodes but i felt like crap. when i found paleo via crossfit the combination of cleaning up food over the time of about 2 weeks or so and crossfitting, i struggled and felt like crap for a bit but i knew when it cleared when i could miss a meal and not feel like i would blackout or pass out. That is the first time in years. good paleo eating has clearly helped and i jsut wanted to share my experience with paleo.
the other interesting part is this is the first time eating in a way that my weight has maintained and stayed the same my gut has flattened but my weight is the same..and for us skinny tall guys that is important to not look emaciated…:) keep up the good work andy and robb and if you ever in KY i’ll treat you to some elk and deer for dinner…afterall your don’t get much more paleo than that.

Mathieu, good afternoon. I apologize for asking you these questions here. I was an attendee at Rob’s Hoboken Paleo seminar this past Saturday and I was not able to ask him a few questions. If I;m consuming 6 oz of protein what is a good gauge as to how much mono/poly fats i should be taking in. Also, is there a certain number of meals that i should be eating on a daily basis. I greatly appreciate any info you can provide.

It all depends on goals! Want to lean out? Little or no fat with that protein. Lean, hard charging athlete? Perhaps LOTS of fat. 2-4 meals seems fine. I bounce between 2-3. Are you able to eat enough at the meals you have to do what you want to do?

Hey Robb, can’t believe I forgot to provide my goals given how long my question is (sorry about that). For now, my goals are to maintain my bodyweight of 150lbs, recover fully from my hamstring injury, and have good energy levels doing upper body workouts (main goal is free-standing handstand). Long-term goals: get stronger and put on some weight by doing another cycle of SS once hamstring is healed. I plan on getting to at least 165lbs, and evaluate performance and body comp when I get there to see what I want to do going further. I know it’s nothing big, but I want to achieve at least a 200lb back squat, 300lb deadlift, and BW power clean (have to work on form).
I weighed myself today and I am at 146-147lbs. The question I meant to ask was that shouldn’t I be eating more given my activity? I slowly lose weight when I try to maintain it, so the answer is I should eat some more, given this weight drop. You pretty much answered my question, thanks for the help and keep up the great work!

Hey Robb… I know you are big believer in taking out the nuts and seeds from your diet.. which I did for about 3 months.. I had great results and got very lean.. started at 185 dropped to 172 (male/6’1″/24/bf% >8).. However I recently read an article from Poliquin about a Rotating Nuts and Meat Breakfast cycle that I am trying.. I am also starting Wendlers 531 strength program to help gain some much needed muscle mass and bigger numbers on my main lifts.. at the same time I dont want to loose my metcon work capacity or my body comp!.. so basically i want my “paleo cake” and I wanna eat it too. Right now im 175# – strict paleo – 100% 50g cartbs a day all from green veggies with a Post Wod meal of Yams (4-5 oz), apple sauce – (3tblsp) and 40g whey protein. this changes depending on my needs. So blocks look like 23 Prot, 75 – 80 Fat, 6 or less CHO.
So my first question..What do you think about this Breakfast Cycle and do you think the following list of nuts are ok to use based on their low omega 6 content – Macadamia Nuts, Almonds, Hazelnuts, pistachios, and cashews. I only eat 2 ounces or less every morning.
Will I see body mass/strength gains on this breakfast…or Will I still see strength gains, and keep my body comp, if I were to take out the nuts and seeds and go with some other fat source.. maybe coconut oil or Avocado’s? Or is the caloric content of these seeds going to help me gain the neccessary body mass to gain strength? And if I do take them out.. do I need to add in some carbs in morning (what types yams or greens?)
I do and have been taking fish oil everyday! grass fed meats blah blah blah.. but just curious as to how this will effect body comp and performance.. I know this question is loaded.. Might be better off as a podcast but any help would be much appreciated!! Love you guys, thanks for providing such a great resource to us all!!

I gave up caffeine on a regular basis about 7 years ago. I read about adrenal fatigue and thought I fit the profile quite well. Energy was crap until a couple years ago when I discovered vit d. Now I do 10K IU per day and use caffeine for “medicinal purposes” only. e.g. If I am going to die while driving because I fell asleep thats medicinal to me. Now I find that I only need caffeine today if I had caffeine yesterday or I got less than 6hrs sleep for a few days in a row.

I think you will be hitting this on your nutrition & supplement pod cast, If so I will wait for the answer, but I need to ask.
I am 43, do cross fit 5 days/ week and have been doing strict paleo for the past 6 months. I have dropped 65 lb since I started (now at 206), I did have a thyroid issue (hot nodule) and had 1/2 of my thyroid removed almost 2 yrs ago. I am not on any medications nor have I ever taken supplements. In the last couple months I have become slightly anemic and think I am low in iodine. I have never had this problem before. I was always told that if I wasn’t careful I would rust and now the blood suckers won’t even take my donated blood. What am I doing wrong? I have tried to eat more leafy greens, but the spinach and chard are now very grainy to me and are hard to eat. Almost like eating dirt. I am eating very low carb and most everything is grass fed. I have also cut back on my coffee to 2 cups/day. I have started taking an iron supplement, but really hate taking pills, I always forget and miss days (thank God I’m not on the pill). Any suggestions on how to fix this would be a great help.

When I looked into it, all the flavors appear to have stevia in them. Is this OK or do I need to go with the unflavored kind? I only ask because artificial sweeteners are not part of my diet now. After the Natural Calm + Zinc becomes routine, I will start to add in some of the other stuff. Thanks

Good Morning Rob,
Currently i’m trying to lean out so i’m assuming that low glycemic carbs along with a lil fat would probably work best however i’m looking to lean out and also harden up. I’m about 5’8″ and currently at 168 so i would like to get down to a lean fit 160. Anything lower than that i begin to look very drawn out. I’m also taking 6 grams of carlson fish oil which has been great because i’m seeing improvements in pretty much all of the wod’s that i’m doing but i’m not sure if it’s too much.. Have you ever come across someone who started to eat Paleo that had irregular bowl movements as this is my case sometimes? Some days i’m solid but most days i’m loose so i’m thinking that i may not be adding in enough fat. I started initially on the Zone and i cut out all grains for the most part during last summer and once I initiated them back into my diet I started to get this lil skin irritation in the crease of my left arm. It normally goes away once i start eating clean again.. So my questions are how much fat and protein (oz’s) should i be taking in if i’m lookin to lean out but harden up, irregular stool patterns and this minor skin irritation on the inside of my left arm when eating grains.. I grew up in an italian household so i’ve been eating pasta’s and breads my whole life but once i cut out the grains and re-introduced them is when i started to get this lil skin irritation. Hi Robb, I failed to notify you that in regards to the loose bowl or irregular bowl movements that on the weekends is when I normally cheat (Beer,Wine and some sort of not so healthy food) and depending on how much alchohol i consume I CHEAT as opposed to cheat, so with this being considered it may have a causative effect on stool patterns. Do you concur?

This whole “pseudo-science” thing is kinda funny at this point. It was originally thrown out as a means of discrediting the paleo concept within crossfit. What it has become is a rallying point for people who “get” that a qualitative approach to food trumps the pseudo-science of 40/30/30 proportionality. CF’s loss at this point.

Thanks Robb. I really appreciate your website and enjoy your podcasts. You’re providing a lot of great support and information for folks traveling down this path. As for your listener base; there are three of us just here where I work (in AZ) who listen to your podcasts.
After 17 years of vegetarianism, always focused on whole foods, I (actually the whole family) started eating some meat a couple of years ago and then transitioned to paleo last October. We’re totally unweighed and unmeasured with lots of grass-fed meats, grass-fed butter, pastured eggs, organic veggies, coconut oil, etc. We look and feel great. The initial impetus to change was reading Weston A. Price’s fantastic Nutrition and Physical Degeneration. The final straw was probably when I read Gary Taubes’s Good Calories, Bad Calories.
Having done what I now believe to be wrong for so many years, I’m very hesitant to proactively promote what I now think is right, but a lot of people seem to ask me anyway. One of the coworkers I work out with (we’ve been doing CrossFit-type workouts for about 3 years) seems to be switching to a completely weighed and measured paleo diet, thus my question about the calorie recommendation.

Robb, Regarding the Gerson Method. I looked at the site a little and found it interesting that he was a physician in pre-war Germany. I am at the part in GCBC where Taubes is talking about the great research that went on in Pre-War II Germany and Austria.
When my daughter, Whitney, had Hodgkin’s Lymphoma when was 13 we were very lucky that she was diagnosed in stage 1. She went through fairly standard chemo (MOPP alternated with another therapy I am forgetting now) and, thankfully no radiation, for 4 months. After talking to her doctor about incorporating some “alternative” treatments, we decided that the lowest impact, like diet, were the most appropriate. While the her doctor was convinced at all about diet having any effect, she thought it was harmless when we told her our plans. I had read some books regarding dietary cures for cancer mostly having to do with macrobiotics, but we just stayed to whole foods as I remember. Paleo like but still some grains. It was strange that she developed a taste for Halibut which she had never liked. Anyway, the point of the story is that near the end of her treatment, she remarked to me that Whitney had weathered the chemo fairly well and that she looked alot healthier than most of her other patients. She admitted that maybe there was “something to the diet thing.”
It is not clear what people are eating before they start a different “curative diet.” The glycemic load from juices could be much less than what they are currently eating making it look like the diet cured the cancer when it was really reducing the “load of work” the body has to do so it can use its own natural healing processes. Really, I think it is like Rip says, “Stronger people are harder to kill.” While it is hard to know exactly what is doing the curing, in the end it may not matter as long as you start as strong as possible.

Have not listened to this podcast yet, but will provide my own anecdotal experience with GERD and Paleo.
Since going Paleo, and with increasing effect as I’ve intensified Paleo, my GERD has slowly disappeared. Used to take Prevacid a few times a week. now don’t need to. I also suspect in my case a 30 pound weight loss (probably 40 pound fat loss) has helped in terms of less abdominal pressure. FWIW

Which brings in the question, I have a client that has a cyst on her vertebrae between her T 8 and T 10, she has been corresponding with a doctor 4 hours away and we would both like to know what information would be the most effective to bring to the doctor. Any Ideas?

Do you have any advice for hard-gainers? My husband has been trying to gain weight since high school and cannot seem to do it. We transitioned to a paleo diet back in October, with him still eating oatmeal every morning and sometimes rice or beans. He is terrified to drop them because he is so lean. He does drink 4 gallons of whole milk per week, but it hasn’t seemed to make much of a difference.
How lean, you ask? The man is 36 years old, 6’4, 165lbs and 3-5% bodyfat. Our calculator might be a bit off, but you can see every muscle, all his ribs (front and back) and veins. I will send you a picture if you want, but he is truly one of the leanest people I have ever seen (don’t call him skinny though…he gets very offended).
Training: We own a small MMA gym, so he does Muay Thai/BJJ with conditioning 4x per week (approx. 75 minutes), heavy weightlifting 1x per week (he can clean well over his bodyweight), light yoga 1x per week (rest day) and swimming 1x per week. We also commute to the gym and work, so he bikes quite a bit. His performance is good, but he really wants to gain weight.

Sorry if this is a dumb question, but it occurred to me a few days ago and I have been curious about ever since. In the past, you have mentioned that consuming liquid foods causes an insulin spike, right? Well, if this is the case, then what about olive oil and coconut milk?

This question is about Paelo Weight loss. I have been doing low carb for about 1 year. I’ve dropped from 277 to 242. Yet due to gi issues I’ve had to switch the nutrition up a bit. I initially came to low carb because of my reactivity to sugars. It got so bad that I would plan being in a coma like state after mealtime (New Yorker but parents are from Dominican Republic so rice, beans, plantains, cassava are the staples). Anyway, energy levels are much much better.
My goals: drop down 50 lbs. Of course, maintain muslce. Currently 242.
I am not deluded about hypertrophy and leaning out. I am willing to lose some strength and muscle, since I am generally strong and could get the strength back up with time.
I am trying to do Pesca-Paleo (I think I coined it). Keeping to the Paleo principles what should I modify in order to promote fat loss. Low-carb was easy but I don’t know if I can maintain 20g a day since I have been doing MEBB and WODs for the last couple of months. I am getting novice effect results for sure. Strength gains are through the roof. But as soon as I do the killer WODS I feel beat for days. I suspect the 20g a day carb is part of the problem. Common sense says I should increase carbs a bit, but whenver I tinker with higher carbs I put on water weight.
What do you think of this approach good: 1g/body weight, 50g carbs, low fat?
P.S. I love me some nuts, but have been cutting back on them.

Unless your friend has cancer which has infiltrated the liver a standard ketogenic diet with high fish oil seems like a better option.

3-Agree with some but not all. Has been kind of a pill to Prof. Cordain for no good reason.

Gluten is a big no no for GERD. I have GERD and so do my sister and father. All of us have had success with no gluten.
My own advice from experience: go very low carb for over a week NO GLUTEN NONE, if you have symptoms take some HCL or ACV, then continue on with a gluten-free diet…forever 🙁
The good news is you shouldn’t cross react with things anymore, so you can enjoy wine, chocolate, and spicy foods!

Know your a believer in espresso beans for your coffee but i was recently made aware that the dark roasting process results in the burning of oils in the bean that can lead some to an allergic reaction. I find myself alot less jitery on light roasted beans (which have a higher caffeine content) then the dark roasts.

ive talked to my doctor and he says its not low blood pressure and doesnt know what else it could be… any ideas?

Meri – If you have Lights Out: Sleep Sugar and Survival go back and skim through the pages because they hit on antihistamines. Can’t remember exactly what they say but antihistamines work on your melatonin and serotonin which seamed dangerous for those who are sleep nazi’s. I have bad allergies as well (use to have VERY bad) but it seems like my allergies has calmed down a bit since I started eating well, my guess in the inflammation that I once had.

About 4ish months ago my friend requested that our dining hall offer olive oil at the salad bar. A few weeks later, they “granted” the request and started putting out a container of “olive oil.” I was totally pumped since I had just started doing Paleo. However, I recently started wondering whether or not the stuff was actually olive oil. They always pour it out of its original container into a clear, plastic bottle, so I had no way of knowing exactly what it was. Today I found out where they keep the stash of “olive oil” and managed to take a look at the ingredient list. Turns out the stuff is like 90% soybean oil! I have been downing this stuff like nobody’s business for the last four months in order to aid in a mass gain. I have been grain, dairy, and legume free (except, apparently for soybean oil…) for the past 4 or 5 months. I have two questions.
1. How badly have I been screwing myself up with all the soy?
2. How long will it take my body to recover from the daily soy overdose? (I recall reading/hearing that it takes the body about ten days to recover from gluten consumption.)
